Ranson's Criteria (Acute Pancreatitis)

Check criteria present at admission and at 48 hours (classic non-gallstone teaching cohort).

Age > 55 years WBC > 16,000/mm³ Blood glucose > 200 mg/dL (11.1 mmol/L) Serum LDH > 350 IU/L AST > 250 IU/L

At first 48 hours (0–6)

Hematocrit decrease > 10% (from admission) BUN increase > 5 mg/dL despite IV fluids Serum calcium < 8.0 mg/dL (2.0 mmol/L) Arterial PaO₂ < 60 mmHg (without lung disease) Base deficit > 4 mEq/L Estimated fluid sequestration > 6 L

Ranson score

Ranson's criteria were developed before modern ICU severity scores; they remain a teaching framework for understanding systemic complications of pancreatitis.

Gallstone pancreatitis uses a modified Ranson set; BISAP, APACHE II, and CT severity index add prognostic information. Arterial blood gas and precise fluid balance may not be available in all settings.
