Weight-based Levothyroxine Dose (Adult Hypothyroidism)

Full oral replacement estimate from body weight for primary (thyroidal) hypothyroidism in adults.

SpO₂/FiO₂ Ratio (S/F Ratio)

S/F ≈ SpO₂ (%) ÷ FiO₂ (fraction). Educational approximation when ABG is unavailable.

Absolute Neutrophil Count (ANC)

ANC (/μL) from white blood cell count and segmented neutrophil plus band percentages.

Calcium Correction for Hypoalbuminemia

Payne formula: corrects total calcium toward a reference albumin of 4.0 g/dL.

PSI/PORT Score (CAP)

Age/sex contribution plus weighted comorbidities, vitals, and labs. Unchecked labs/exam items are treated as “not abnormal” for the Class I screen (teaching default).

Framingham Risk Score (10-Year Hard CHD)

Point-based 10-year risk of hard coronary heart disease (angina not included in “hard” endpoints in original tables). Uses published ATP III / Framingham worksheet: total cholesterol bands depend on age decade; diabetes is not added as points (ATP III treats diabetes as a CHD risk equivalent separately).

HEART Score

Sum of five domains (each 0–2): History, ECG, Age, Risk factors, Troponin — total 0–10.

MELD and MELD-Na

Bilirubin (mg/dL), INR, creatinine (mg/dL), sodium (mEq/L); dialysis checkbox forces creatinine term to 4.0 (teaching).

4Ts Score (HIT)

Thrombocytopenia, Timing, Thrombosis sequelae, oTher causes — each scored 0, 1, or 2 points (max 8).

Kocher Criteria (Pediatric Hip)

Each item +1 (max 4): inability to bear weight, fever ≥38.5°C, ESR ≥40 mm/h, WBC ≥12,000/mm³.

qSOFA Score

One point each: respiratory rate ≥22/min, altered mentation, systolic BP ≤100 mmHg (max 3).

mMRC Dyspnea Scale

Select the statement that best describes breathlessness related to usual daily activity.

Stanford Sleepiness Scale

Rate how likely you are to doze in the stated situation (1 = no chance, 7 = high chance). This form uses a single aggregate item for teaching.
