PHQ-9 is a nine-item depression module aligned with DSM-IV/DSM-5 major depressive symptoms, widely used in primary care and research as a severity measure and response tracker.
Limits
Positive item 9 (self-harm thoughts) requires clinical triage regardless of total score. PHQ-9 is not a stand-alone diagnosis; bipolar, grief, substances, and medical illness can mimic depression.
