SOFA quantifies dysfunction across six organ systems and is used in Sepsis-3 organ dysfunction teaching (change from baseline context in practice).
This form does not store baseline SOFA deltas, FiO2 when PaO2 unavailable, or bilirubin in µmol/L conversion. Pediatric modifications are not modeled.
