Hemodynamically unstable (sBP <100 and HR >100, ICU need, or clinician judgment) Thrombolysis or embolectomy indicated (other than hemodynamic instability) Active bleeding or high risk for bleeding >24 hours supplemental oxygen required to maintain SaO₂ >90% PE diagnosed while patient is on therapeutic anticoagulation Severe pain requiring IV analgesia for >24 hours Medical or social reason for admission >24 hours (e.g., infection, malignancy, no support) Creatinine clearance <30 mL/min (Cockcroft–Gault) or severe liver impairment

How it works

Hestia identifies hemodynamically stable PE patients who may be treated as outpatients when no exclusion criterion is present.

Limits

Does not apply to unstable patients; pregnancy, HIT, and other exclusions may exist outside this 8-item teaching list.
